Complaints


The Practice will take seriously any complaint recieved from members of the public and will acknowledge all complaints within 3 working days of receipt.

If you would like to request a copy of the Practice's Complaints procedure, please enquire at reception, or alternativley email: info.collierswood@nhs.net. Please note the Practice has the right to charge for such requests.


If you would like to make a complaint you can download the Patient Complaint Form document. The document consists of guidlines for how to make a complaint to the Practice, a complaint form, and a third party consent form if you wish to complain on behalf of someone else.

Alternatively you can use our Suggestions/Comments section to make your complaint if you wish.
